MDG 1: Integration, Food Security and HIV/AIDS

The links between food security, good nutrition and HIV&AIDS are well known, including the fact that good nutrition supports positive living for people living with HIV who do not yet need antiretroviral therapy (ART), and that good nutrition is essential for people who are on ART. However, good nutrition is only possible when people have secure access to nutritious food. HIV and AIDS can undermine food and income security, increase the risk of HIV transmission, decrease resistance to opportunistic infections, undermine access and adherence to treatment, and exacerbate social and economic impacts of the disease. Clearly, food and nutrition security are extremely important in the push towards universal access to prevention, treatment, care and support for people living with HIV (PLHIV).
And yet the "how to" of responding to these issues is not always so clear given their complex and multifaceted nature. Taking action leads to four fundamental challenges, specifically:
1. Challenges inherent with integrated programming;
2. Finding middle ground between advancing science & promulgating promising practices;
3. Tailoring a response to meaningfully involve those infected/affected by HIV and AIDS; and
4. Building field practitioner capacity for integrated programming.
How these challenges are dealt with will determine the impact and success of integrated programs.
Challenge 1: Integrated Programming
There are inherent challenges associated with ‘integrated programming'. The idea that practitioners want to or have the means to work together across sectors is not a given. A major factor contributing to this is stove piped funding by sector, making it difficult to design and implement fully integrated programs. Organizational structures often reflect funding patterns with specialized staff focusing on their sector rather than the integration of interrelated sectors. These staff or units within organizations often do not talk to each other, and as a result do not understand what the other is doing. While this is beginning to slowly change, many development agencies still do not have effective linkages or collaboration between different departments given that they are sometimes housed in separate buildings, are accountable to different people and have a completely different way of viewing the world. Bringing integration to life and/or making it happen is a significant challenge that the development community must be mindful of; integration will not happen on its own.
Challenge 2: Finding Middle Ground Between Advancing Science & Promulgating Promising Practices
Development organizations like Project Concern International (PCI) view action as evidence. Currently many innovations are taking place on the ground as implementers grapple with the ‘how to' of integrated programming. Where field practitioners experience breakthroughs in integrating their work across sectors, what can be done to make sure that donors, international organizations, policy makers, and others become aware of these innovations in "real time" and are able to learn from them? How do we keep a pulse on what is going on at the field level while simultaneously bringing lessons learned from the research/policy level to the field? The development community needs to identify a better way to merge the elements of policy and evidence, both research and operational. There is a need to pinpoint the middle ground between project reports and published research that would enable us to capture promising practices and disseminate them promptly and efficiently, both to policy makers and field practitioners, in language they can both understand.
Challenge 3:Meaningfully Involving those Infected and Affected
Africa Forum 2009: Sharing Integrated Solutions to HIV and Food & Nutrition Insecurity led by PCI took place in June, 2009 in Mangochi, Malawi. Approximately 170 Africans from 18 sub-Saharan African countries participated in a practitioner-led, non-academic conference AF09 was the second event of its kind; the first took place in Zambia and was called Africa Forum 2006 (AF06): An Integrated Response to the Dual Epidemics of HIV/AIDS and Food Insecurity. The AF06 Declaration laid the foundation for AF09 in Malawi, outlining key priorities for future integrated programming, including the need to: 1) make the integrated programming a priority; 2) increase learning and information sharing in integrated programming; and 3) ensure that decision-making about integrated programming is guided by the voices of those most affected.
AF09 participants came to Malawi with high levels of passion and commitment and were excited about what was a first opportunity for many to not only attend such an event, but to actively participate as presenters, panelists, and/or site-visit or skills-building facilitators. AF09 participants had varied HIV status, represented diverse fields, and came from different organizations from local NGOs to national policy setting bodies. Over the course of the week the enthusiasm of participants seemed to grow rather than wane, despite the feeling of being overwhelmed at times with so many hands-on activities. Participants left AF09 strongly motivated to do things differently, such as consistently including the voices of those most affected, and using dynamic methods to share knowledge.
The challenge beyond AF09 is how to establish and/or strengthen platforms to meaningfully involve affected communities and PLHIV in decision-making. Creating a bridge between policy, research, programmatic implementation, and those who are most affected by the work, is a challenge to be aware of and understand how to channel for optimal effect.
Challenge 4: Building Practitioner Capacity for Integrated Programming
In order to ensure that field staff will apply innovative approaches and ultimately bring them to scale, we must invest in their capacity to do so. PCI is building capacity around integrated programming by facilitating peer-based learning, founded on adult education and behavior change principles. Adult learners, especially field staff, need to be actively engaged for learning to take place; they require dynamic skills-oriented learning approaches. During AF09 and AF06, each participant was expected to both a learner and a contributor. This meant that each participant had to lead a session in one form or another, as well as to actively participate in hands-on educational activities. Participants learned by getting their hands dirty, for example digging gardens, preparing food, and visiting community projects. Disseminating reports and traditional training approaches are not enough to stimulate change; adults need active learning methods to bring about behavior change. This needs to be acknowledged and incorporated into development programming.
Moving Forward
In order to take effective action on integrated programming, we must contend with a number of challenges. First, in order to bring integration to life and make it work, we need to overcome the lack of sufficient funding streams for integrated programming. We also need to find the middle ground between advancing science and promulgating promising practices; more collaboration needs to happen to bring the best of both worlds to bear for maximum synergies and mutually beneficial results.
The development community must also tailor responses to meaningfully involve communities and those infected & affected by HIV&AIDS. Finally, in order to implement high quality programs that have been proven to work, we must build the capacity of field practitioners, introducing them to new skills through vibrant, innovative and creative learning opportunities.
In dealing with these challenges it is important to recognize that no one solution or mechanism exists to deal with these issues; there is no "silver bullet." However, as shown through AF09 and AF06, networking, collaboration, learning and scaling up integrated programming will happen if encouraged, benefiting those who need it most.

I am puzzled by O’Donnel-Blake’s spin on MDG 1 which seems off target and to lack a sense of scale.
MDG I is about eradicating extreme poverty and hunger and MDG includes halting the spread of HIV. Fortunately MDG 6 has been achieved in the incidence of new infections is falling in India and many other places. MDG I is not being achieved in subSaharan Africa and parts of Asia.
In 2007, the UK All Party Group on population, development and reproductive health took evidence from international agencies and experts from around the world and concluded that “The MDGs are difficult or impossible to achieve” without slowing rapid population growth. Fortunately even poor and illiterate women will have fewer children when offered contraception and accurate information backed up by safe abortion.
Niger is an example of country with rapid population growth. Today there are 15 million people and 2050 there is likely to be over 50 million, There is no plausible way the country can feed itself. Education will never catch up with growing numbers of children. To talk about ‘integrated’ programs when, for example, a mega disaster is unfolding across the Sahel is fanciful in the extreme.
